Virtus Atlanto-Axial Instability (AAI) Clearance Form for Athletes with Down Syndrome

For athlete safety this form is required for athletes with Down Syndrome who have applied for Virtus or Paralympic eligibility. The form requires the doctor’s signature as well as an attached neck x-ray/scan which many already have -(though a new neck scan may need to be arranged) which you should discuss this with your doctor.

E-signing is recommend as Athletes Without Limits will receive it without an additional transfer needed.
